The Tahltan/Stikine Community-Led Primary Care Network (PCN) is a collaborative partnership between the Nation, the Pacific Northwest Division of Family Practice, and Northern Health. The vision of Indigenous Community-Led Primary Care Networks is to transform primary care to ensure timely and accessible integrated team-based care within community.
Reporting to the Health Director (or designate) and the Regional Manager, Coast Mountain Primary Care Network, the Far North PCN Coordinator provides a community focused approach in the coordination of the partnership, planning, implementation, and evaluation of the PCN work within the context of Culturally Safe and Respectful practices.

Key role responsibilities:
- Provides administrative support to the Health Director, PCN steering committee and other committees as assigned on projects and initiatives.
- Supports the development and implementation of project work plans, coordinates project activities, and identifies strengths and gaps related to the implementation of the PCN.
- Supports member and stakeholder engagement activities.
- Organizes and facilitates meetings; works with the partners on agenda setting, prepares and distributes meeting material, identifies, tracks, and follows up on action items.
- Provides community information about PCN implementation activities and outcomes and communicates progress of the PCN with community, Leadership, CM PCN Regional Manager, partners and the PCN Steering Committee.
- Develops resources for community members and the public, including manuals, communication briefs, and emails.
- Collaborates with colleagues to streamline and optimize operational processes and procedures.
- Works closely with the PCN evaluation team to coordinate PCN data collection. This includes gathering timely data collection to meet Primary Care Network requirements.
What You Bring:
- A background in Health Care, Health Information Management, Business Administration, or a related field and 3-5 years of experience or an equivalent combination of education and experience.
- Established relationships within the Far North Indigenous Communities and ability to navigate service, administration, and leadership relationships preferred.
- An understanding of the unique cultural practices, protocols, beliefs, and values of the Far North Nations preferred.
- An understanding of the BC health system, primary care, and client-centred care.
- An understanding of the Social Determinants of Health and its impact on health and well-being, specifically to those if Indigenous Ancestry.
- Experience managing complex projects from conception through to evaluation.
- Proven ability to work with and adjust approaches to meet the needs of diverse teams, (for example: Government, Health Care professionals, Community Service Partners, and community members).
- Experience with gathering, basic collation and communicating complex information and data.
- Demonstrated experience in community engagement, change management and action planning.
- Skilled at collaborating diverse information from diverse team members to prepare briefing notes, background information, agendas, for effective meetings.
- Ability to remain flexible in the midst of evolving priorities, yet clearly communicate boundaries.
- A passion for improving community health and well-being while supporting service provider satisfaction.
- Proficiency in Word, Excel, Outlook and PowerPoint.
