The Prince Rupert – Coast Ts’msyen Primary Care Network (PCN) is a collaborative partnership between Coast Ts’msyen communities, the Pacific Northwest Division of Family Practice, and Northern Health. The vision of Primary Care Networks is to transform primary care for patients and providers, to ensure every interested patient can access longitudinal integrated team-based care in their Patient Medical Home.
Title: Prince Rupert Coast Ts’msyen PCN Project/Change Coordinator – Clinic Start-up

Position Compensation: $45/hr – 37.5hrs/week
Position Term: Contract 3 – 6 months
POSITION DESCRIPTION:
This position is funded and supported through the PCN (Primary Care Network) Steering Committee. You will be employed by the Pacific Northwest Division of Family Practice, reporting to the Community Service Manager – Northern Health Authority.
You will work to transition a private primary care clinic to a Northern Health operated clinic, aligning with Northern Health policies and procedures, as well as Ministry of Health directives and PCN Mandates.
Using strong project coordination and change management expertise, you will act as the connector and problem solver across multiple tasks, initiatives, and stakeholder groups. You will guide the timely completion of safety upgrades, workflow redesign, and operational improvements. A key part of this role is fostering collaboration and trust with physicians, clinic staff, and partner leadership throughout the transition process.
