Medical Outreach Clinic: Outreach Nurse

Location: Terrace, BC
Employment Type: Full-Time or Part-Time
Contract: 1-Year Term (Possible Extension)
Reports to: Clinical Director or Designate
Salary: Up to $105,000 annually. Nisga’a Valley Health Authority offers comprehensive benefits for employees who join the organization at 0.5 FTE or higher. JOB SUMMARY:

The Terrace Medical Outreach Clinic is a collaborative initiative within the Coast Mountain Primary Care Network with BC Ministry of Health, Pacific Northwest Division of Family Practice and Nisga’a Valley Health Authority. The clinic uses an integrated case management service model to address the complex primary care needs of marginalised communities living in Terrace. Recognising the impacts of the social determinants of health on the health of individuals, the service team takes a collaborative approach in navigating individuals to services, resources and supports.

In alignment with the vision and values of the Coast Mountain Primary Care Network, the Medical Outreach Clinic (MOC) team members use culturally safe and trauma informed approaches in the provision of care.

The Outreach Nurse practices according to the standards of professional practice and code of ethics established by the BC College of Nurses and Midwives (BCCNM). This role operates within a longitudinal, team-based, and place-based primary care service model.
The Medical Outreach Nurse provides direct primary care and case management while also assuming a leadership role in coordinating, supervising, and offering clinical expertise. The role facilitates collaborative problem-solving as a key member of the multidisciplinary team.

This position ensures high-quality, client-focused primary care by assessing, treating, and managing cases across the full spectrum of MOC services while navigating clients to their Most Responsible Primary Care Providers (Physicians and Nurse Practitioners).

In collaboration with the Medical Outreach Clinic Team, Community Advisory Group, and Implementation and Quality Assurance Committees, the Outreach Nurse supports the planning, implementation, coordination, and evaluation of program outcomes.

TYPICAL DUTIES & RESPONSIBILITIES:

1. Provides direct care and case management services to LBOC clients using decolonized and trauma-informed approaches, ensuring culturally safe and respectful place-based care.
2. Supports client enrollment in the Provincial Attachment System and facilitates navigation to the appropriate primary care provider, focusing on MRP care and attachment.
3. Assesses the learning needs of staff, provides coaching, and contributes to the development and implementation of in-service education and professional development programs.
4. Provides input to the Medical Director and Implementation Committee on the development and implementation of clinical standards and operational procedures, ensuring they align with the needs of vulnerable individuals, professional standards, evidence-based practices, and accreditation requirements.
5. Delivers clinical services, performing client assessments, identifying, and planning place­based primary care strategies, implementing care plans, and evaluating treatment outcomes.
6. Documents assessments and professional judgments using the appropriate EMR, including reports, referrals, and other confidential documents.
7. Leads clinical referral pathways to internal and external care providers and ensures follow-up on internal and external referrals.
8. Assists the Medical Director or designate in measuring the quality of care delivered within the program by providing reports and reviews for the Medical Director, PCN Steering Committee, or Ministry of Health.
9. Promotes a psychologically safe and healthy workplace by maintaining competencies in Non-Violent Crisis Intervention and other relevant employee wellness initiatives.
10. Assists in staff orientation through one-on-one coaching and instruction.
11. Supports the Medical Director in ensuring the availability of necessary resources, including staffing, supplies, and equipment, in collaboration with team members.
12. Supports the evaluation of appropriateness, organization and supervision of practicum placements for students from recognized institutions and training programs related to service prov1s10n.
13. Performs other related duties as required.

REQUIRED EDUCATION, TRAINING, AND EXPERIENCE:

• Bachelor’s degree in nursing.
• Minimum of four years of recent related direct care experience,
• Registration with the BC College of Nurses and Midwives (BCCNM).
• Willingness to complete Cultural Safety Training, Trauma-Informed Practice, and other decolonization skills training.
• Valid BC driver’s license.
• Applicants with or pursuing Remote Practice Certification will be given highest priority.
