Salary: Hourly rate starting at $102.10

JOB SUMMARY:

The Terrace Low Barrier Clinic is a collaborative initiative within the Coast Mountain Primary Care Network, in partnership with the BC Ministry of Health, the Pacific Northwest Division of Family Practice, and Nisga’a Valley Health Authority. The clinic uses an integrated case management service model to address the complex primary care needs of marginalised communities living in Terrace. Recognising the impacts of the social determinants of health on the health of individuals, the service team takes a collaborative approach in navigating individuals to services, resources and supports.

In alignment with the vision and values of the Coast Mountain Primary Care Network, the Terrace Outreach Clinic (TOC) team uses culturally safe, trauma-informed approaches in the provision of care.

The Nurse Practitioner practices according to the standards of professional practice and code of ethics established by the BC College of Nurses and Midwives (BCCNM). This role operates within a longitudinal, team-based, and place-based primary care service model.

The Nurse Practitioner (NP) provides comprehensive direct primary care, including assessment, diagnosis, treatment, prescribing, and ongoing case management. In addition to delivering full-scope clinical care, NP plays a key leadership role within the interdisciplinary team, contributing clinical expertise, supporting coordinated care planning, and strengthening collaboration to ensure high-quality, patient-centred services.

In collaboration with the Terrace Outreach Clinic, Community Advisory Group, and Implementation and Quality Assurance Committees, NP supports the planning, implementation, coordination, and evaluation of program outcomes.

Full scope primary health care services, including but not limited to:

  • Health promotion and illness prevention
  • Chronic disease management
  • Primary mental health and substance use services
  • Primary care for minor or episodic illnesses
  • Management and coordination of patient care
  • Primary reproductive care
  • Participation in multidisciplinary team planning

Clinical administrative services, including but not limited to:

  • Health service planning
  • Participation in evaluation of services
  • Reporting

REQUIRED EDUCATION, TRAINING, AND EXPERIENCE:

  • Master’s degree in nursing (Nurse Practitioner designation)
  • Clinical experience in direct patient care
  • Registration with the BC College of Nurses and Midwives (BCCNM)
  • Willingness to complete Cultural Safety Training, Trauma-Informed Practice, and other decolonization skills training
  • Valid BC driver’s license
