Description                                     

The Haida Gwaii Primary Care Network (PCN) is a partnership between Pacific Northwest Division of Family Practice, Northern Health Authority, First Nations Health Authority, Niislaa Naay Healing House Society, XaaydaGa Dlaang Society and local communities. The vision of PCN is to transform primary care for patients and providers, to ensure every interested patient can access longitudinal integrated team-based care in their Patient Medical Home.  Position Summary

The PCN coordinator supports the collaborative partnership and PCN goals established by the Haida Gwaii PCN Steering Committee in providing culturally safe, team-based care by Health professionals within Haida Gwaii communities.

The PCN Coordinator brings a wealth of coordinator support to the Haida Gwaii PCN implementation team. Following the vision set by the Haida Gwaii Primary Care Network Steering Committee and the operational Service Plan for the PCN as approved by the Ministry of Health and the Family Practices Services Committee, this position will be responsible for coordinating and implementing assigned related activities. As part of this work, this position also plays a critical role in successfully operationalizing the Haida Gwaii PCN. The role works with physicians, Northern Health Authority, First Nations Health Authority, and First Nation communities, and other stakeholders to increase access to quality primary care in Haida Gwaii.

Key Duties & Responsibilities

 Provides support to the PCN steering committee and other committees assigned on projects and initiatives.

  • the development and implementation of project work plans, coordinates project activities, and identifies strengths and gaps related to the implementation of the PCN.
  • Supports member and stakeholder engagement activities
  • Organizes and facilitates meetings; works with the partners on agenda setting, prepares and distributes meeting material, identifies, tracks, and follows up on action items.
  • Provides reports on PCN implementation activities and outcomes and communicates progress of the PCN as directed by the Haida Gwaii Co-Chairs.
  • Develops resources for members and the public, including manuals, communication briefs, and emails.
  • Collaborates with colleagues to streamline and optimize operational processes and procedures.
  • Works closely with the PCN evaluation team to coordinate PCN data collection. This includes gathering timely data collection to meet Primary Care Network requirements.
  • Ability to adapt to changing workload priorities, effectively reprioritizing or deferring tasks in line with operational and strategic goals.
  • Supports the coordination of reporting and invoicing

 Knowledge, Skills, and Abilities

  • An understanding of the BC health system and primary care. Experience working directly in healthcare or supporting healthcare initiatives is an asset.
  • Experience invoicing and basic financing
  • Demonstrated understanding and application of quality improvement methodologies, ideally in health care settings, including the ability to understand the linkages between workflow and structured data capture and meaningful reports, extract and utilize measures, and evaluate success of the PCN
  • Experience and training in change management principles to guide awareness of the people side of change to achieve PCN success.
  • Skilled at preparing for effective meetings.
  • Demonstrated ability to handle multiple projects, and works effectively with deadlines, and balance work priorities in changing healthcare landscape.
  • Committed to cultural safety and to continuous cultural learning and to lifelong learning.
  • A passion for improving primary care and supporting Primary Care Networks to enhance patient care and provider satisfaction.
  • Skilled at navigating a variety of personalities and social circumstances – knows when to adjust communication styles to meet the emotional state or cultural climate with an understanding of the dynamics within a practice team, or committee
  • Proficiency in Word, Excel, Outlook and PowerPoint.
  • Travel within the region will be required.
