Coast Mountain Primary Care Network (PCN) Coordinator (0.5-1.0 FTE)

The Coast Mountain Primary Care Network (CM PCN) is a collaborative partnership between Tahltan, Nisga’a, Kitselas, Kitsumkalum communities, the Pacific Northwest Division of Family Practice, and Northern Health. The vision of Primary Care Networks is to transform primary care for patients and providers, to ensure every interested patient can access longitudinal integrated team-based care in their Patient Medical Home.

Reporting to the Regional Manager, Coast Mountain Primary Care Network, and supporting the Primary Care Network Partners, the PCN Coordinator provides administrative supports that align with the PCN goals established by the Coast Mountain PCN Steering Committee.  

The PCN Coordinator brings a wealth of administrative support coordination support to the PCN Implementation Team. Following the vision set by the Primary Care Network Steering Committee and the Service Plan for the PCN, this position will be responsible for coordinating and implementing assigned related activities. As part of this work, this position also plays a key role in successfully supporting the data reporting requirements of the Coast Mountain PCN.  Respect and knowledge of First Nations cultural principles and protocols is a key success factor for the PCN Coordinator role.

Key role responsibilities:

  • Provides support to the PCN steering committee and other committees as assigned on projects and initiatives.
  • Supports the development and implementation of project work plans, coordinates project activities, and identifies strengths and gaps related to the implementation of the PCN.
  • Supports member and stakeholder engagement activities.
  • Organizes and facilitates meetings; works with the partners on agenda setting, prepares and distributes meeting material, identifies, tracks, and follows up on action items.
  • Provides reports on PCN implementation activities and outcomes and communicates progress of the PCN as directed by the Coast Mountain PCN Co-Chairs.
  • Develops resources for members and the public, including manuals, communication briefs, and emails.
  • Collaborates with colleagues to streamline and optimize operational processes and procedures.
  • Works closely with the PCN evaluation team to coordinate PCN data collection. This includes gathering timely data collection to meet Primary Care Network requirements.

What You Bring:

  • A background in Health Care, Health Information Management, Business Administration, or a related field and 3-5 years of experience or an equivalent combination of education and experience.
  • An understanding of the BC health system, primary care, and client-centred care.
  • An understanding of the Social Determinants of Health and its impact on health and well-being.
  • Experience managing complex projects from conception through to evaluation.
  • Proven ability to work with and adjust approaches to meet the needs of diverse teams, (for example: Government, Health Care professionals, Community Service Partners and community members).
  • Experience with gathering, basic collation and communicating complex information and data.
  • Demonstrated experience in community engagement, change management and action planning.
  • Knowledge, understanding and the application of progressive quality improvement methodologies, ideally in health care settings.
  • Skilled at collaborating diverse information from diverse team members to prepare briefing notes, background information, agendas, for effective meetings.
  • Ability to remain flexible in the midst of evolving priorities, yet clearly communicate boundaries.
  • Committed to the ongoing development of competencies in cultural safety, reciprocity and reconciliation.
  • A passion for improving community health and well-being while supporting service provider satisfaction
  • Proficiency in Word, Excel, Outlook and PowerPoint.
