SUPPORTING CHANGE BC ACROSS THE PROVINCE

Our collaborative partners have been supportive in helping us make the CHANGE  Program one of the cornerstones as we develop our Primary Care Network (PCN) model. The CHANGE Program can be a socially responsible, culturally appropriate, scalable program for the entire province, starting right here in the Pacific Northwest. CHANGE BC is a bold step towards delivery of a pro-active healthcare program which is the first of its kind across the country.

Prescribing the CHANGE Program has changed the lives for so many of our patients and we are excited to make it available to even the most vulnerable people across the northwest.

Currently, we offer the CHANGE BC Program in Terrace, Houston, and the Nisga’a Valley. If you are part of the Pacific Northwest Division of Family Practice but do not have access to the CHANGE Program in your community, please contact Colleen Enns to request support in setting up this program.

If you are interested in starting the CHANGE Program within your PCN and are outside of the Pacific Northwest, our team can support you in getting started.

Support for starting the CHANGE Program in other Divisions of Family Practice

The Pacific Northwest Division of Family Practice freely shares the following services and resources:

  • Support for program and Primary Care Network service planning
  • Family physician and allied health team orientation and training
  • Program templates, resources and materials
  • Network with an engaging team of colleagues, including Family Practice and Academic linkages (locally, provincially, and nationally)
  • Access to evaluation resources including the CHANGE Evaluation Matrix, developed collaboratively with Healthcare Excellence Canada to recognize CHANGE BC as a promising practice nationally 
  • EMR and data management solutions
  • Liaising with experienced project managers and clinicians delivering the CHANGE Program
