The Outreach Service

• Primary care with integrated case management for marginalized communities in Terrace
• Services are currently offered from a mobile clinic at local shelters, supported living complexes and encampments
• Stationary clinic space is being established to complement the mobile service
• Operating in strong collaboration with community and health authority partners

The Outreach Team

The outreach team work out of the mobile clinic, partner sites and the soon to be established stationary low-barrier clinic, with offset schedules to ensure at least two staff at any service delivery point, 7 days per week.

• Clinical Director and Clinic Manager
• Nurses
• Physicians
• Social Workers
• Mental Health and Substance Use Clinicians
• Cultural Navigators
• Peer Support Workers

Nursing Roles and Schedule

• Schedule Direct care and case management for individuals who rarely access primary care
• Work within full scope of Remote Practice Certification (if qualified) with physician support
• Adequate time for comprehensive assessments
• Supported by the multidisciplinary team
• Help improve community health outcomes and reduce ER congestion
• Operating hours: 10:00 a.m. – 6:00 p.m., 7days/week
• Flexible commitments available: full-time, part-time, casual

Compensation

• Hourly rate: $56 – $62, depending on qualifications and experience
• Comprehensive benefits package for 0.5 FTE and higher (prorated based on FTE)
• Travel and accommodation covered
