Virtual Care Coordination

The Pacific Northwest Division of Family Practice provides VIRTUAL CARE COORDINATORS to help connect physicians and patients for virtual visits.  Our aim is to increase access to care for our rural, remote, and Indigenous community members across the northwest by providing Virtual Care Coordinators (VCCs) to support video enabled calls between a patient, their primary care provider, and/or a specialist/specialized service provider.

Virtual Care Coordinators are part of a Virtual Care Coordination team available to support all Pacific Northwest Division of Family Practice communities in the following ways:

  • Assists patients who have difficulties accessing virtual care, haven’t used virtual technology, or do not have access to the Internet.
  • Provides patient resources and training for their virtual appointment and helps patients to access connectivity.
  • Works to ensure family practices receive timely communication and follow up back to their offices.
  • Supports the coordination of virtual visits with the patient’s health care team.
  • Works with Pacific Northwest communities to create additional linkages with Primary Care Networks as they become operational.
